MIRIAM STOPPARD I know, from the many letters I receive on the subject, that living with erectile dysfunction is difficult for men and their partners. ![]() It can make sex impossible - or just awkward and embarrassing. Powerful drugs such as Viagra have been an enormous help for many of the UK's estimated 2.3 million men with difficulties but, because the effects only last four to six hours, users often complain it takes the spontaneity out of sex. However a less well-known, but I think more useful, drug called Cialis has helped ease this problem as it has the power to last for up to 36 hours. As a result, sex can be more relaxed and doesn't have to be pre-planned. A fresh approach... Although Cialis is not brand new - I first wrote about it more than five years ago - it has only just been licensed as a once-a-day tablet. Men can now take the pill as part of their morning routine and then forget about it, just like a woman taking her contraceptive pill. I think this is a huge step forward, as research shows not having to pop a pill in a premeditated way makes men feel sexually normal again, rather than like a furtive teenager. The same research found that three-quarters of men with erection problems hated having to schedule sex. And nearly 80 per cent thought sex after taking shorter-lasting impotence treatments was less exciting. Another factor that impresses me about Cialis is that it could help beat erectile dysfunction in more than half of men who have nerve damage caused by diabetes - a common trigger for erection problems - according to one study. In my mind, these two facts give Cialis a distinct advantage over Viagra, or indeed Levitra (a similar drug which lasts for around five hours), and put it in a class of its own. What's the cause? Most men will experience erection problems at some point in their life due to anxiety, tiredness or too much alcohol. But, if the occasional problem starts happening more often, it could be the sign of an underlying health condition. To get a strong erection, you need good blood vessels and nerves to the penis. Conditions such as diabetes and cardiovascular disease can have a physical effect while emotional or psychological issues are also a trigger. Problems at work or money worries can have an impact on performance and sexual confidence can be very difficult to get back once it is lost. Who will benefit from taking Cialis daily? Men who want to enjoy sex at least once a week - without having to timetable it - will benefit. Although Cialis works for up to 36 hours, this doesn't mean a man has an erection for this long - it only works when he is sexually stimulated. Taking the pill daily means never having to worry about the effect wearing off too quickly, so sex can be spur of the moment. Is it safe for me? If you suffer from hypertension (high blood pressure), heart conditions, bleeding disorders, liver problems or peptic ulcers, or have had a stroke, any sexual performance drugs could cause serious side effects so you must not take them without advice from your GP. As with Viagra, men taking heart medicines containing nitrates in any form shouldn't take Cialis, to avoid a potentially dangerous drop in blood pressure. Nitrates include the heart medicine nitroglycerine - GTN - and the long-acting nitrates used to treat angina. Cialis is generally well-tolerated but side effects can include headache and indigestion. Less common are facial flushing, stuffy nose, muscle aches, dizziness and backache, all generally mild to moderate and short-lived. Should I buy Cialis on the internet? Some men are tempted to buy drugs such as Cialis or Viagra on the internet, but this is effectively gambling with your life. Not only are such tablets often fake, or contaminated with toxins, but unprescribed erectile dysfunction drugs can have had adverse medical reactions such as heart attacks. Taking a powerful drug without a medical check-up is asking for trouble. Why you must get it checked If you develop erection problems, you must overcome any embarrassment and get them checked out by a doctor - it could just save your life. Erectile dysfunction can be an early warning for diabetes or heart disease, as these conditions affect circulation, reducing blood flow to the penis. Left untreated, you're putting yourself at risk from a heart attack or stroke. WORKING TOGETHER If your partner is having erection problems, being patient and loving is vital. Here are a few supportive tips to help him overcome the problem... TAKE THE PRESSURE OFF It's extremely important to take the stress out of sex. Your man will be feeling very anxious if he's struggling to maintain an erection, so forget about penetration and stick to kissing, massaging and stroking each other. Just enjoy each other's bodies. REASSURE HIM He's likely to be terrified about being unable to perform so let him know how much you love him and that it's no big deal and something you can tackle together. PICK YOUR MOMENT CAREFULLY The first important step when tackling impotence is to make sure your man isn't over-tired. Tiredness can kill an erection so, if you know he's had an exhausting day, wait until a calmer moment such as at the weekend. TIME IT RIGHT Many men find they can get an erection in the morning when it's impossible at night, so grab him before the alarm goes off. TRY A NEW POSITION The tried-and-tested missionary position can be quite tiring for a man. Lying side by side or spooning, with the man behind, is far less work leaving him free to concentrate on enjoying the moment. GIVE IT TIME No two men will respond in the same way to medication so you have to be patient. It may take several attempts before he can get an adequate erection. Take your time - and enjoy trying. LIKE many men suffering from erectile dysfunction, Steven didn't tellanyone about his problem. The 44-year-old financial lawyer was too embarrassed. After months of making excuses to his wife, he decided to take action. Butinstead of going to see his doctor, he turned to the internet. ![]() 'I felt ashamed,' says Steven. 'I know I should have gone to see the doctor butI couldn't even admit to my wife what was happening, let alone tell a man Ihardly knew.' He found an internet pharmacy that sold Viagra withoutprescription. 'The testimonials seemed convincing.' Steven paid around [pounds sterling]20 withhis credit card, and a few days later 16 tablets arrived by post; thedistinctive blue pills were sealed in blister packs labelled '100mg sildenafilcitrate', the active ingredient in Viagra. There was no box or information leaflet. Steven took one an hour before bed, as advised by the website. 'It wasincredible. I was able to resume a normal life with my wife,' he says. 'Ofcourse, I didn't tell her about the tablets. I placed a second order for morepills straight away.' However, with the second batch he immediately noticedsomething different. 'Within half an hour of taking one tablet I had athrobbing headache and my face went bright red. There was also the desiredeffect, but I felt so awful I had to sit in the bathroom until the headachesubsided about an hour later and then went to bed. 'I couldn't understand why it had this effectthe tablets looked identical to the last ones, and the blister packs were thesame. A few days later, I tried again and had a similar experience. This time I also felt nauseous.' Steven finally admitted everything to hiswife, who persuaded him to see his GP. 'From my reaction, the doctor said it was likely I'd bought counterfeit drugswith a much higher dose of active ingredient than was safe,' says Steven.Symptoms of a Viagra overdose include chest pain, nausea, irregular heartbeatand feeling light-headed or fainting. His GP said other patients had described similar problems. Steven was thenprescribed a drug similar to Viagra and was advised on ways to reduce stress (apossible cause of erectile dysfunction). 'I feel like an idiot now,' says Steven. 'I put my health at risk because I wastoo embarrassed to seek help.' Steven's experience is far from unusual. Theglobal trade in counterfeit medicines is worth [pounds sterling]10billion a yearand while drugs for impotency and hair loss are popular targets, fake statins,blood-clotting drugs and even cancer medicines are also being pushed on to anunsuspecting public and healthcare professionals. Criminal gangs are now so sophisticated that they buy the same packagingequipment as drug companies and even attend trade fairs, explains EricNoehrenberg, of the International Federation of Pharmaceutical Federation of Pharmaceutical Manufacturers and Associations. 'It makes it difficult, if not impossible, to tell true products from fakes,except through extensive chemical tests,' he says. 'Britain is a lucrativemarket for counterfeiters. Sometimes people take medication and it doesn'twork. Nobody thinks to ask what was actually in their pills and whether they weregenuine. But if you are unlucky, you will get serious health effects.' COUNTERFEIT medications tested by the Medicines and Healthcare ProductsRegulatory Agency (MHRA) have found traces of brick dust and rat poison, saysMick Deats, head of enforcement for the drugs watchdog. 'Some contain no active ingredients at all; others have much higher doses thanis safe. This is a major health risk.' Until now, it's the Third World that hasreally suffered from drug counterfeiting. Half the malaria medicines in use inAfrica, for instance, are thought to be fakes. But over the past two years, theproblem has become endemic in Europe, too; in 2005 there were half a millionsingle doses of fake medicines discovered in Europelast year that figure shot up to 2.5million. Only last month, a Hertfordshire businessman was jailed for four-and-ahalfyears for his role in a multi-million pound trade in fake Viagra. AshishHalai, with his fellow conspirator Gary Haywoodwho is awaiting sentencinghad smuggled the pills into the UK from China, India and Pakistan, thendistributed them to customers via the internet. Undercover officers filmed Haywood trying to sell fake drugsincluding Cialis, a drug for impotency, and Propecia, for male patternbaldness. He claimed he was a pharmacist and international sales manager for Viagramanufacturer Pfizer and had got the drugs 'on the cheap'. Haywood had paid as little as 25p each for a tablet, and charged up to [pounds sterling]20.When officers visited his home in Barwell, Leicester, they discovered boxes ofcounterfeit pills worth around [pounds sterling]1.5million. The investigationled to another 12 convictions in America, with [pounds sterling]13 million of counterfeit pillsseized in Miami. However, experts believe this is the tip of the iceberg. The World HealthOrganisation (WHO) now estimates that up to 50 per cent of drugs bought onlineare fakes. Yet the problem is not confined to the internet, says Jim Thompson, chairman ofthe pressure group the European Alliance for Access to Safe Medicines. Halai and Haywood had also managed to get some of the counterfeit products onto chemist shelves; 10 per cent of all prescription medications globally arecounterfeits, says the WHO, and this figure is set to grow.. 'There is no doubt that fakes have entered the legitimate supply chain,' saysMr Thompson. Counterfeits can often contain some active ingredients. However, some may have more than the recommended dose, which puts people atrisk of overdosing. 'Counterfeiters don't necessarily want to harm patientsif someone dies or become ill, it raises suspicion and they don't want to getcaught. So the drugs simply may not workwhich is also risky for the patient.' On three separate occasions between 2005and 2006, the MHRA found fake versions of the cholesterol-lowering drug Lipitorbeing sold, unwittingly, by pharmacists. The counterfeits were so sophisticated they carried forged security codes onthe packs, impossible to distinguish from the genuine article. But they werefound to contain an unlicensed statin. To identify the fakes, Pfizer, thelegitimate manufacturer of Lipitor, was forced to recall 120,000 packs of itsdrug. This summer, drugs manufacturers had to order a recall of three drugs,Plavix (to prevent blood clotting), Casodex (to treat prostate cancer) andZyprexa (an anti-psychotic) after fakes were unwittingly handed out bypharmacists. Mr Thompson believes it's impossible to gauge the true scale of the problem.Between 1995 and 2005, the MHRA tested around 25,000 packs of medicines,randomly sampled from pharmacy shelvesless than 0.001 per cent of prescriptions handed out over that period. 'We simply don't know how many fakes there are out there,' says Mr Thompson.'No one is even looking.' Criminals get into the legitimate supply chain bytaking advantage of the fragmented way drugs are traded across the EuropeanUnion. People can buy and sell medicines like any other commodity, says Julian Mountfor Pfizer Pharmaceuticals Worldwide. 'The same rules that govern the tradingof soft drinks and jeans apply to pharmaceuticals. It's a free-for-all atpresent and not safe for the supply of medicines.' European law allows licensedtraders to buy and sell medicines freely. This means that once a medicine hasleft the manufacturer's gates, it can be sold onand repackagedany number of times before it reaches a pharmacist. DRUGS in the UK are moreexpensive than in other European countries; dealers can make a profit by buyingcheaply in Greece, for example, then selling the drugs to UK pharmacists. Because patient information leaflets must match the native language of thecountry where the drug is sold, it is permissible for the product to berepackaged by third parties. This provides a way for counterfeiters to enter the legitimate supply chain. Pharmacists don't realise they are buying fakes. As David Pruce, of the RoyalPharmaceutical Society of Great Britain, explains: 'They are victims, too. Inmany casesincluding the Lipitor casefakes have been sold to them by reputable wholesalers, who had unwittinglybought batches of counterfeits.' The profits are huge and the risks are 'verysmall compared with trafficking narcotics', says Mr Noehrenberg. 'With theexpansion of the EU, the threat is only increasing.' Mr Thompson says it is aproblem both the MHRA and the Government need to address. 'They have no controlover what is going on and they won't even admit it's their system that's atfault.' However, Mick Deats, of the MHRA, believes the systemall traders are licensed by the MHRAis safe, and that reports of counterfeits being sold to pharmacists and the NHSare greatly exaggerated. 'There have only been nine known occasions that counterfeits have ended up onpharmacy shelves, and in just four of those cases a licensed wholesaler wasinvolved.' Of course, the real need is to tackle the problem of fake drugs atsource. Seven illegal factories in China were closed as a result of the Viagra scam. However, the general view is that the manufacture of medicines is poorlyregulated in countries such as China and India where fake drugs oftenoriginate. As Mr Deats explains: 'We can only pass on information to regulators in othercountries and hope they act on it.' Victor Chen, the Chinese-based manufacturerwho supplied Gary Haywood with drugs, is still at large. 'Neither we nor our counterparts in China have any idea where he is,' says MrDeats. 'There would be nothing stopping him from setting up shop under a newidentity.' And plying his wares on even more unsuspecting consumers. 'As a patient, you have the right to insist that you receive theexact same drug from the same manufacturer and dealer every time,' says MrThompson.
